Read through the entire document and make changes that apply to you, your practice and state regulations/guidelines. Some of the sections have been left intact so you can see what it may look like once you’re finished.
Add what is lacking under your circumstances and remove whatever does not apply to you.
Add all other practice guidelines that apply to you. For instance, this template does not include practice guidelines for multiples nor breech. Feel free to add them. You may also consider including all of your consent forms in this document.
We found it best to leave the contract and financial agreement out of this document. The home birth practice guidelines handbook is something you will be able to give to prospective clients; ideally after a complimentary consultation.
Your contract and financial agreement can be on hand – ready to share with the prospective client – in case all parties feel compatible.
For printing, make sure the first page of each section is an odd page number. Bind in whatever method is best suited for your needs. We found a 3-ring binder worked well as it has the potential to become a document well over 100 pages.
